Club Affiliation System
The PLMHA uses the Club Affiliation system. Therefore, on the official PLMHA team lists only the players assigned to the team show on the team list. Affiliated players are not added.
Coaching Clinic Reimbursement Authorization Form
Coaching clinics will only be reimbursed to applicants who have been approved by the PLMHA Executive prior to taking a course. Refer to the Coaching Clinic Reimbursement Policy for additional information.
Team Travel Permit from ODMHA (Out of branch)
This form must be filled out for a team traveling to an out-of Branch tournament/exhibition series. Hockey Canada Registration #s are found on the teams official roster. Along with the completion of the top section, all players, affiliates and team officals must be listed with #s, name, full address, jersey # or team official position, and birthdates.
