Procedure for Making Game Changes

As your team's coach or manager you will want to make changes to your game schedule, either because you need to clear a day or two for an away tournament, or because the other team has asked for the game change. Either way, it is very important to follow the correct  procedure with both your league and association. The Ice Scheduler and the Referee in Chief must be notified of the change to avoid having your team face fines to recover wasted ice or referee salaries. There are step-by-step instructons here to help you do things properly. Please read them even if you are an experienced coach or manager. Some procedures have changed or you may have forgotten a step or two from years past.


RESPECT IN SPORT

If you will be a coach, assistant coach, trainer or manager then you must complete the Respect in Sport - Leader program, which is available here.
 


Vulnerable Sector Check
All volunteera with the PLMHA must have a Vulnerable Sector Check on file with the association and must redo the check every two years. Print this letter and get it signed by a member of the PLMHA executive.

 



NEW REQUIREMENT FROM HEO: Gender Identity Training

All team officials must take the following two free online courses on gender equality:

There are two courses which will educate team officials on this important topic - Understanding Discrimination based on Gender Identity and Gender Expression and A Guide to the Dressing Room Policy and Confidentiality Statement.
All coaches and trainers must take these two courses and coaches will be awarded 3 certification maintenance points for taking the courses. HEO recommends that all team officials and administrators take this training given its significant benefit. For team officials already assigned to a team, completion of these modules will be required by October 1, 2017.

Gender Equality Dressing Room Policy

If you will be coaching or managing in the PLMHA then you should read and be familiar with the Gender Equality Dressing Room Policy, which has been adopted by Hockey Eastern Ontario.
 



Travel Permits

If you are registering for a tournament within ODMHA, you do not need a travel permit.  If you are registering for a tournament OUTSIDE the ODMHA, then you must complete a travel permit.  This process should be completed 4-6 weeks prior to the tournament (or as soon as possible).  Do not leave this until the last minute.

  1. Complete the Team Travel Permit available HERE.

  2. Once completed, submit to the PLMHA President and the PLMHA registrar

If you are registering for a USA tournament.  You must complete the USA Team Travel Permit form available HERE.   

Please note that the maximum insurance coverage for out of country is $5000.  Parents should consider purchasing supplementary insurance for out of country events.


Code of Conduct

The signed Code of Conduct must be completed by every player/family registered with Perth Lanark Minor Hockey.  The team manager is responsible for ensuring that this is complete and available upon request by an Executive member.

Website Release Form

The website release form should be completed by every player/family registered with Perth Lanark Minor Hockey Association,  The team manager is responsible for ensuring that the form is completed and readily available upon request.

AFFILIATION FORM

New for the 2023-24 season, ONLY rep teams are required to fill out the applicable affiliation form.  House league teams will complete affiliations using the TTM portal.
For rep teams, this form must be completed in full (player information and signature, parental signature, affiliated players coach signature and affiliating team coach signature).  Once completed, it is to be submitted to the association registrar to sign and submit to District for approval.  Once all approvals have been obtained, the player will be added to the affiliating teams official roster.  This process must be completed before player can affiliate.

If a house team is needing to affiliate for a tournament, the affiliated player MUST be added to the official roster, therefore you are required to notify the association registrar for them to be added.  Using TTM is ONLY for league games.

**Note that no player is permitted to miss their own team event to attend an event with the affiliated team, without prior approval from their own coach.  Failure to follow this requirement can result in disciplinary action to both the player and the coach of the affiliated team.**
